I unknowingly feel it works in all positions, but you mainly feel the pleasant pressure when lying on your back, as the largest possible surface area of the body is under load (it puts a lot of pressure on the legs); it's not as noticeable when lying on your side.
As a 100kg man, I blindly chose the 9kg version as the golden mean recommended online; I could have easily handled more.
