Unfortunately, it doesn't fit the iP 18 Pro! There was a gap between the case and the phone at the top, which meant the phone wasn't held securely in the case and dirt could get in (see photo). This might be due to slightly different dimensions of the camera module between the iP 17 Pro and 18 Pro. Apple itself doesn't state that cases are compatible between these models.

If you have a screen protector, it's flush with the protector. So if you drop it, you'll smash the screen protector at most. Of course, the dimensions for the 17 pro vs the 18 pro are the same. But the camera module is higher on the 18 pro, so you can use this on a 17 pro without any issues, but the cover will stick out about 1mm more above the lenses than the original one for the 17 pro. But in my opinion, that's not a bad thing at all. Quite the opposite.
